Нужна PHP Mailer Form со скриптом File Attachment для отправки информации формы с вложением или без него

У меня возникли проблемы с попыткой заставить мой скрипт отправить вложение файла с информацией, полученной из формы. Я использовал PHP Mailer Form с вложенным файлом и изменил, чтобы соответствовать моей спецификации. До сих пор он будет отправлять информацию о форме без прикрепленного изображения, но когда я загружаю изображение, он будет отправлять только вложение и никакой другой информации из формы. Мне нужно, чтобы иметь возможность отправить информацию формы с вложением или без него. Я новичок в php, поэтому я не уверен, зачем он это делает.

Этот код был опробован и протестирован, поэтому он должен работать. Также посмотрите, установлен ли sendmail. Если в системе Linux Ubuntu попробуйте sudo apt-get install sendmail, чтобы установить его.
