extJS как выставить параметры через кнопку?

У меня есть этот код. Если я нажимаю на кнопку, мои параметры формы вставляются в базу данных. Как я могу это сделать? что я должен написать там? И если я нажму на форму сброса, мое окно закрывается. Пожалуйста помоги! Спасибо!
