«Данные ManagementForm отсутствуют или были изменены

Я использую следующий код в моем приложении django, который работает нормально, если я использую его как есть, но когда я расширяю базу до следующего html, он выдает следующую ошибку:

django .core.exceptions.ValidationError: ['Данные ManagementForm отсутствуют или подделаны']
